ALL THE ITEMS LISTED BELOW CAN BE FOUND ON THE FORMS PAGE UNDER MISCELLANEOUS APPLICTIONS FORM 284-238
ALL DOCUMENTS REQUIRED FOR THESE PERMITS MUST BE UPLOADED THROUGH THE PUBLIC PORTAL AND ARE SUBJECT TO REVIEW PRIOR TO ANY APPROVAL OF A FINAL PERMIT.
ALL PERMIT TYPES LISTED BELOW ARE MANDATORY SELF-CERTIFICATION AND MUST BE SIGNED OFF BY A LICENSED CONTRACTOR.
